Information required by Registrar to register a death
When someone dies, there are numerous local authority and government departments that need to be informed. Devon County Council offer a service to help achieve this called, "Tell Us Once."
If the doctor certifying the death has sought a Coroner's Referral or, if a Post Mortem is required, the relevant papers will be sent by the Coroner directly to the Registrar.
Please telephone the Registrar to confirm receipt of the forms before visiting the Registration Office.
You will need:
The Registrar will ask for a form of identification of the deceased. This could be the deceased's Medical Card, Pension Book (if applicable), Passport or Driving Licence. It would be useful to take along the deceased's Birth and Marriage Certificates. The Registrar can take the majority of information from them and thus, ease the procedure.
Having collected the Medical Certificate stating the Cause of Death and The Formal Notice from the Health Authority, take these with you to the Registrar's and decide how many copies (purchasable at £4.00 each), of the Death Certificate you require.

YOU WILL BE GIVEN:-
A white form – not the Death Certificate - this form is for Social Security (DWP) purposes for cessation of State Pension payments.
2 A green form
It is most important to hand this form to your chosen Funeral Director as soon as possible. It is the AUTHORITY for the Funeral to take place. The Funeral Director must submit this form to the Medical Referee at your chosen Crematorium at least 48 hours prior to the Funeral.
Where a 'post mortem' is carried out, the Coroner's Office WILL need to know the funeral/cremation location.
For a burial where no inquest will follow, the registration MUST be completed before the funeral or the interment CANNOT proceed.
